Hospice Admission Nurse

We are seeking a compassionate and skilled Hospice Admission Nurse to join our team at LifeBridge Health.
Job SummaryAs a Hospice Admission Nurse, you will play a critical role in ensuring that patients receive high-quality, patient-centered care during the final stages of life. Your primary responsibility will be to assess patients' and families' needs and develop individualized plans of care that meet their unique needs and preferences.
Key Responsibilities- Conduct thorough assessments of patients' physical, emotional, and spiritual needs
- Develop and implement individualized plans of care that address patients' needs and preferences
- Collaborate with the interdisciplinary team to ensure continuity of care and effective communication
- Provide education and support to patients and their families to promote self-care and independence
- Document patient progress and outcomes in accordance with regulatory requirements
- Associate's Degree in Nursing from an accredited institution
- Minimum of two years of recent medical/surgical nursing experience, with one year of home health experience preferred
- Expertise in performing health assessments and developing individualized plans of care
- CHPN certification within four years of hire or after meeting CHPN exam requirements
- CPR certification from the American Heart Association
