We are seeking a compassionate and skilled Hospice Admission Nurse to join our team at LifeBridge Health.
Job SummaryThe successful candidate will be responsible for ensuring that all services are delivered in accordance with the Mission Statement and S.P.I.R.I.T. Values of LifeBridge Health. This includes providing professional, comprehensive, family-oriented nursing care to individuals in their home setting during the final stages of life.
Key Responsibilities- Assess patients' and families' needs and formulate, implement, and evaluate a plan of care.
- Communicate with the hospice interdisciplinary team to ensure continuity of care.
- Provide expert health assessments and develop individualized care plans.
- Maintain accurate and timely documentation records.
- Associate's Degree in Nursing from an accredited school.
- Minimum of two years of recent medical/surgical nursing experience.
- One year of home health experience preferred.
- CHPN - Certified Hospice Palliative Nurse certification.
- CPR - AHA Healthcare Provider certification.
- RN - Registered Nurse licensure.
