As a Hospice Admission Nurse with LifeBridge Health, you will play a critical role in ensuring that patients receive high-quality, compassionate care in their final stages of life. Your expertise will be essential in assessing patients' and families' needs, formulating and implementing plans of care, and communicating with the interdisciplinary team to ensure continuity of care.
Key Responsibilities- Assess patients' and families' needs and develop individualized plans of care
- Provide professional, comprehensive, family-oriented nursing care to patients in their home setting
- Communicate effectively with the interdisciplinary team to ensure continuity of care
- Maintain accurate and timely documentation of patient care
- Collaborate with other healthcare professionals to ensure seamless care transitions
- Associate's Degree in Nursing from an accredited school
- Minimum of two years of recent medical/surgical nursing experience
- One year of home health experience preferred
- Expertise in performing health assessments
- Current RN licensure in the state of Maryland
- Bachelor's Degree in Nursing
- CHPN certification
- CPR certification
