We are seeking a highly skilled Clinical Pharmacist to join our dynamic team at Northside Hospital. The successful candidate will be responsible for providing clinical pharmacy services, serving as a resource for staff development, and participating in quality initiatives.

Responsibilities
  1. Assess and monitor patient medication regimens to ensure optimal care.
  2. Develop and maintain effective working relationships with key stakeholders, including physicians, mid-level providers, case managers, and nursing personnel.
  3. Participate in quality initiatives, evaluating and analyzing medication use practices to improve patient outcomes.
Requirements
  • Graduate of an accredited School of Pharmacy.
  • Doctor of Pharmacy, or Master's Degree with completion of a residency in the assigned clinical area or equivalent experience.
  • Two (2) years previous hospital pharmacy experience.
