POSITION OVERVIEW

The Patient Care Coordination Specialist is responsible for administrative tasks and facilitating communication within a healthcare unit to support patient care initiatives.

PRIMARY RESPONSIBILITIES
  • Conducts administrative tasks essential for patient care activities. Initiates, organizes, and maintains patient records following established protocols. Archives reports and test outcomes efficiently.
  • Manages the processing of prescribed tests, medications, procedures, and appointments. Oversees patient admissions, transfers, and discharges while keeping team members informed of reports, orders, and relevant information.
  • Handles incoming calls and oversees the patient communication system. Acts as a reliable information source for patients, staff, and the public, ensuring appropriate interactions with individuals of all ages.
  • Completes and maintains unit and patient documentation, including files, forms, and records pertinent to operational management and patient care, such as admissions, surgeries, transfers, discharges, medical records, and physician contact lists. Ensures an organized workspace for records, files, forms, and reference materials. Regularly checks, updates, and maintains facility boards and related systems according to departmental policies and guidelines.
  • Prepares reports, purchase orders, projects, memos, and correspondence utilizing computer applications.
  • Welcomes visitors and provides assistance as required. Manages phone communications and contacts nursing and medical staff through various devices. Monitors response times for patient call lights and assigns communication devices to appropriate personnel.
  • Performs designated tasks as instructed by nursing or medical staff. Runs errands, collects supplies, and assists in patient transportation.
  • Maintains inventory levels, including requisitions/forms, and ensures the work area is stocked with necessary supplies and equipment.
  • Ensures a clean, safe, and functional workspace.
  • Coordinates patient transportation and related items as assigned.
KEY ATTRIBUTES FOR SUCCESS
  • Exceptional interpersonal skills and the ability to comfort patients; appears approachable, friendly, and reassuring.
  • Ability to follow directives from medical professionals.
  • Familiarity with medical documentation and records.
  • Strong attention to detail.
  • Basic computer proficiency and keyboard skills.
  • Completion of a Health Unit Coordinator Training Program is preferred.
