- Continuous Care Coordination: Ensure seamless care delivery by coordinating with healthcare providers, patients, and families to address changing needs and concerns.
- Documentation and Reporting: Maintain accurate and detailed clinical notes, progress reports, and other documentation to support patient care and quality improvement initiatives.
- Education and Licensure: Current RN licensure in the state of practice and one year of clinical experience.
- Certifications and Training: Current CPR certification and completion of any required training programs.
