Care Management RN

At CHRISTUS Health, we are seeking a skilled and compassionate Care Management RN to join our team. As a Care Management RN, you will play a vital role in providing high-quality patient care and ensuring seamless transitions between healthcare settings.

Key Responsibilities:
  • Develop and implement individualized care plans that address the unique needs and goals of each patient.
  • Collaborate with healthcare teams to ensure comprehensive care coordination and communication.
  • Provide education and support to patients and their families to promote self-management and empowerment.
  • Monitor and evaluate patient progress, making adjustments to care plans as needed.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements:
  • Bachelor of Science Degree in Nursing, preferred.
  • 1 year of experience in the related nursing specialty, preferred.
  • Critical Care/Emergency Department experience, preferred.
  • BLS required.
  • RN License in state of employment or compact.
  • Current PALS certification at time of hire or within 12 weeks of hire for experienced RNs or completion by the end of the clinical orientation period for new graduate nurses.
  • Current ACLS certification at time of hire or within 12 weeks of hire for experienced RNs or completion by the end of the clinical orientation period for new graduate nurses.
  • Current TNCC and ENPC certification within 12 months of hire for experienced RNs or completion within 18 months of hire for new graduate nurses.
  • Current NIHSS certification at time of hire or within 90 days of hire for experienced RNs or completion within 12 months of hire for new graduate nurses.
  • Modular Sedation within 3 months of hire for experienced RNs or within 12 months of hire for new graduate nurses.
  • De-escalation training within 90 days of hire and maintain training as indicated per program.
  • Completion of an evidence-based triage curriculum at: 12 months of hire for new graduate or inexperienced RNs (less than 1 year of nursing experience) 3 months of hire for experienced RNs (more than 1 year of nursing experience)
  • Current Arrhythmia recognition at time of hire or within 8 weeks of hire for experienced RNs or completion within 6 months for new graduate nurses
