We are seeking a compassionate and skilled Registered Nurse to join our Hospice team as a Case Manager. As a valued member of our team, you will play a critical role in delivering high-quality end-of-life care to our patients and their families.

Key Responsibilities
  • Conduct thorough initial nursing evaluations to determine patient eligibility for hospice services.
  • Develop and implement personalized plans of care that address the physical, psychosocial, and environmental needs of our patients.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination.
  • Provide education and support to patients and families to promote self-care and independence.
  • Maintain accurate and detailed clinical records to ensure continuity of care.
Requirements
  • Current RN licensure in the state of practice.
  • One year of clinical experience, preferably in hospice care.
  • Current CPR certification.
  • Valid driver's license and reliable transportation.
