Key Responsibilities:
1. Develop and coordinate a holistic interdisciplinary care plan tailored to each patient’s needs, integrating their preferences and values.
2. Conduct ongoing assessments and evaluations to monitor patient progress and adjust care plans as necessary.
3. Advocate for patients by identifying and addressing barriers to care and ensuring access to necessary resources.
4. Collaborate with supervisors and care transition teams to facilitate effective implementation and continuous evaluation of health plans.
5. Serve as a resource for accurate medical record documentation and provide recommendations for levels of care and post-discharge services.

Qualifications:
Licensure / Certification / Registration:
- Current Registered Nurse license from the appropriate state board or a multi-state license (Compact State) is required.
- BLS certification from the American Heart Association or American Red Cross is preferred.
- Case Manager certification from the Commission for Case Manager Certification (CCMC) or the American Case Management Association (ACMA) is preferred.
Education:
- Diploma from an accredited nursing school or equivalent professional licensure at the time of hire.
