About the Position

Your Key Responsibilities
  1. Providing direct patient care and support in post-acute facilities, including tracking patients' wellbeing, physical and mental health, and vital signs.
  2. Collaborating with healthcare teams to develop effective treatment plans, conducting intakes, documenting health concerns and medical history.
  3. Administering and verifying medications, monitoring their effects, and reporting any adverse reactions or concerns.
  4. Maintaining accurate and detailed medical records, reviewing residents' charts and adhering to HIPAA guidelines.
  5. Participating in ongoing training and professional development, staying up-to-date with the latest medical techniques and best practices.
