Job Description:

We are seeking a highly skilled Nurse Practitioner to join our team at UnitedHealth Group. As a key member of our Senior Community Care product, you will work closely with a talented team to provide exceptional care to patients in their homes, nursing homes, and assisted living facilities.

Responsibilities:
  • Care Delivery: Provide high-quality, cost-effective care to assigned members, effectively managing medical and behavioral conditions.
  • Assessments and Documentation: Perform comprehensive assessments, documenting findings in a clear and concise manner compliant with CMS regulations.
  • Medication Management: Conduct initial medication reviews, annual medication reviews, and post-hospitalization medication reconciliations.
  • Care Coordination: Facilitate agreement and implementation of the member's plan of care, collaborating with facility staff, families, primary care physicians, and specialty care providers.
Requirements:
  • Certifications and Licensure: Certified Nurse Practitioner through a national board, active and unrestricted license in the state of residence, current DEA licensure/prescriptive authority (or ability to obtain post-hire).
  • Experience and Skills: 1+ years of hands-on post-grad experience in Long Term Care, proficiency in electronic medical records, ability to develop and maintain positive customer relationships.
