About the RoleThe Licensed Practical Nurse in Home Health is responsible for providing high-quality patient care and performing technical skilled care in the patient's home. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities- Provide quality patient care and perform technical skilled care in the patient's home.
- Develop and implement individualized care plans in collaboration with the healthcare team.
- Continuously observe and report patient's nursing needs to the supervising nurse and/or physician.
- Instruct patients and their families in the care of the patient and maintenance of a healthy environment.
- Current LPN licensure in the state of practice.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
- CPR Certification required.
