Job Title: Interim Housing Case Manager

LA Family Housing is seeking a highly motivated and compassionate Interim Housing Case Manager to join our team. As a key member of our Programs - Interim Housing division, you will play a vital role in assisting participants with assessing and connecting to community resources.

Key Responsibilities:
  • Assess participant needs and develop individualized plans to address goals, such as income, medical, mental health, substance use, education, childcare, and social support.
  • Provide ongoing strength-based case management services and coordination with participants.
  • Offer referrals, linkages, information, and support to connect participants with needed resources.
  • Assist participants in obtaining necessary documentation for housing connections.
  • Collaborate with internal and external providers to ensure effective care coordination.
  • Maintain accurate records using written forms and HMIS/CHAMP.
  • Conduct crisis and risk assessments, and develop safety plans as needed in consultation with supervisors.
  • Provide crisis intervention and conflict management support when necessary.
  • Follow mandated reporting guidelines, communicating with protective and emergency services as required, and completing follow-up documentation, including incident reports.
  • Attend regular staff, agency, and community meetings as directed by supervisors.
  • Stay informed about best practices and interventions related to Domestic Violence, Harm Reduction, Trauma-Informed Care, and the Housing First Model.
  • Demonstrate flexibility in schedule and site locations, as needed for multiple sites.
Requirements:
  • At least one (1) year of relevant work experience.
  • Experience in dealing with substance abuse, mental health, and crisis intervention desired.
