Job Summary:

Serves as a key facilitator for the pharmaceutical care team, fostering a culture of excellence in patient care. Participates in ongoing education and training of team members, ensuring they possess the necessary skills to deliver high-quality services. Coordinates quality improvement initiatives, promoting a safe, efficacious, and cost-effective use of medications in assigned patient populations.

As a seasoned pharmacist, you will leverage your expertise to drive positive change within the organization. Your ability to effectively communicate and collaborate with team members will be essential in achieving our goals.

Responsibilities:

  • Facilitate pharmaceutical care team meetings and activities
  • Develop and implement quality improvement initiatives
  • Provide education and training to team members
  • Ensure compliance with Ohio pharmacy law and regulations
  • Collaborate with patient care teams to deliver exceptional patient outcomes

Requirements:

  • Doctor of Pharmacy degree
  • Licensed or license eligible by the Ohio State Board of Pharmacy
  • Completion of residency training in pharmacy practice or three years of increasingly responsible experience in a pharmaceutical care position
  • Experience in Oncology Practice (Certification preferred)
