Position Overview
The Patient Care Coordinator plays a vital role in overseeing the administrative and discharge planning processes for all patients. This position is essential in ensuring that patients receive comprehensive care tailored to their needs.

Key Responsibilities
  • Develop and implement Continuing Care Plans in collaboration with patients.
  • Facilitate weekly psycho-educational sessions to support patient recovery.
  • Assist patients in completing necessary documentation for Short Term Disability and FMLA.
  • Accurately initiate, explain, and complete the Referral and Intake Processes.
  • Ensure the collection and delivery of Exit Surveys to the appropriate staff for data input.
  • Maintain confidentiality in all patient interactions and adhere to privacy regulations.
  • Provide a warm welcome and guidance to patients and visitors.
  • Respond promptly and professionally to all inquiries via calls and mail.
  • Organize and maintain an efficient filing system.

Qualifications
The ideal candidate will possess a high level of interpersonal skills, demonstrating poise, professionalism, and diplomacy in all interactions. Key qualifications include:
  • Excellent organizational abilities with a strong capacity to prioritize tasks and meet deadlines.
  • Proficiency in various computer software applications, including MS Word, Excel, and PowerPoint.
  • A deep understanding of diverse cultures and gender-specific issues, with the ability to address the needs of varied groups.
  • Commitment to maintaining confidentiality at all times when engaging with patients, families, and other stakeholders.
  • Exceptional customer relations skills, characterized by supportive and constructive communication.

Education and Experience
A Bachelor's degree in psychology, social work, or a related field is preferred. Candidates should have a minimum of two years of experience in a mental health or healthcare setting. Experience with insurance verification and billing is advantageous.
