Job Summary

VITAS Healthcare is seeking a compassionate and skilled Home Health Aide to provide personalized care and support to patients in their homes. As a Home Health Aide, you will work closely with our interdisciplinary team to deliver high-quality care and improve the quality of life for our patients and their families.

Key Responsibilities
  • Provide personal care and assistance with daily living activities, such as bathing, dressing, and grooming.
  • Assist patients with meal preparation and feeding, as needed.
  • Support patients with ambulation and mobility, including transfers and positioning.
  • Perform various treatments and procedures, as ordered by the physician and supervised by the nurse, including colostomy care and range of motion exercises.
  • Monitor and record vital signs, including temperature, pulse, and respiration.
Additional Responsibilities
  • Assist in maintaining a safe and healthy environment for patients.
  • Encourage patients to participate in activities that promote mental alertness and engagement.
  • Provide emotional support and companionship to patients and their families.
  • Report any changes in patient condition or home situation to the VITAS Nurse and interdisciplinary team.
  • Teach basic personal care skills to primary caregivers.
Requirements
  • High school graduate or equivalent preferred.
  • Meets personnel qualifications of a Home Health Aide as specified in 42 C.F.R. Section 484.4 and Section 484.36.
  • Documented evidence of 75 hours of training as specified in 42 C.F.R. Sections 484.36 (a)(1).
  • Documented evidence that Home Health Aide has successfully completed a Competency Evaluation program as described in 42 C.F.R. Section 484.36(b).
