About the Role

We are seeking a skilled and compassionate Home Health Aide to join our team at CenterWell Home Health. As a Home Health Aide, you will play a vital role in providing personalized care to patients in the comfort of their own homes.

Key Responsibilities
  • Provide direct patient care under the supervision of a registered nurse and according to the patient's plan of care.
  • Assist patients with self-administered medications, ensuring accurate dosing and observation.
  • Take vital signs, including temperature, pulse, and blood pressure, and report any abnormal results to the supervisor.
  • Support patients with daily living activities, such as bathing, grooming, and skin care.
  • Assist with feeding patients, promoting nutrition and hydration.
  • Help patients with mobility, exercises, and positioning, ensuring their safety and comfort.
  • Document patient interactions, care rendered, and any changes in the patient's condition.
Requirements
  • High school diploma or equivalent.
  • Completion of a Certified Nursing Assistant or Certified Home Health Aide program within the last 24 months.
  • Meet applicable state certification requirements.
  • A valid driver's license, auto insurance, and reliable transportation.
  • At least one year of experience as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, or home health/hospice agency.
