Job Overview

Crossover Health is seeking an onsite Advanced Primary Care Provider for our Pepsico Primary care Clinic in Purchase, NY. We are open to full or part-time M-F 8am-5pm. (This is an onsite position)

Job Responsibilities

The Advanced Primary Care Provider is responsible for meeting all clinical, administrative, and financial performance metrics. This position implements, delivers, and ensures the highest quality of patient-centered care.

  • Provides comprehensive primary care services
  • Participates in selecting suitable treatment plans for patients
  • Supervises staff to ensure the care team provides appropriate care according to their duties and obligations
  • Assists other care providers and extended care teams
  • Offers continuous availability for medical and technical questions to employees, patient care staff, and ancillary care providers, including after-hours coverage
  • Takes an active role in quality improvement programs and participates in multidisciplinary quality assurance programs
  • Collaborates with the management team as needed to improve patient care
  • Cooperates with clinical outcome reviews and measurements
  • Complies with all policies, procedures, and protocols
  • Provides general medical care and treatment to patients under the direction of a physician (depending on state)
  • Submits healthcare plans and patient goals for periodic review and evaluation by physicians
  • Provides after-hours coverage in rotation with other market clinicians
  • Prescribes or recommends drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures
  • May refer patients to physicians for consultation or to specialized health resources for treatment
  • May provide occupational health services (post-offer physicals, occupational surveillance exams, medical monitoring, return-to-work exams, and Worker's Compensation medical guidance) in some health centers
  • May need to perform therapeutic procedures such as injections, immunizations, suturing, and wound care as necessary
  • If applicable, delivers care at any clinic during scheduled work hours when necessary
  • Performs other duties as assigned
Required Qualifications
  • Minimum of 3 years of clinical experience in Internal Medicine, Family Practice, or Occupational Health
  • Must be Board Certified
  • Must be licensed to practice in the appropriate state
  • BLS certification required
