The Grady Health System is seeking a highly skilled Nurse Navigator to enhance the patient experience in Breast Imaging. This role involves providing navigation services from screening mammograms to diagnostic services and onto the treatment process.
Key Responsibilities- Collaborate with Breast Imaging radiologists and the interdisciplinary team to serve as the primary point of contact for patients and families in Breast Imaging.
- Coordinate and manage patient care in Breast Imaging through scheduling, supporting, educating, advocating, providing a link to research, addressing barriers to care access, and providing indirect patient care.
- Monitor and coordinate scheduling, patient tracking, procedure education, and follow-up care to ensure completion of recommended care.
- Provide image-guided biopsy results when appropriate and hand off to Breast Specialist and Oncology RN Navigator as needed.
- Help increase access to Breast Imaging by working with community navigators to arrange scheduling of needed mammograms.
- Work in collaboration with patient scheduling, radiologists, social workers, and others to determine and address barriers to care.
- Coordinate and manage data reporting and tracking in systems such as EPIC, MagView, and other systems to demonstrate reduction of barriers and case management.
- Bachelor's Degree in Nursing is required.
- Master's Degree in Nursing is preferred.
- Three (3) or more years of direct nursing experience and demonstrated clinical competence.
- Or more years case management experience is preferred.
- Current licensure as a Registered Nurse in the State of Georgia is required.
- Basic Life Support (BLS) certification is required.
- Demonstrate competence in clinical specialty.
- Utilize advanced clinical thinking and analytical skills that contribute to patient adherence of treatment protocols and symptom management.
- Competent in use of Electronic Medical Records.
