About the Job: As a MDS Coordinator at Sodus Rehab & Nursing Center, you will play a critical role in ensuring the accuracy and timeliness of Minimum Data Set (MDS) assessments, care plans, and supporting activities.

Responsibilities:

  • MDS Assessment and Care Planning: Ensure that all MDS assessments, care plans, and supporting activities are completed accurately and on time, promoting the best possible outcomes for residents.
  • Monitoring and Compliance: Monitor the completion of MDS assessments by other disciplines within the prescribed timeframe and ensure the accurate completion of MDS/RAPs/Triggers sheets.
  • Care Plan Implementation: Implement care plans and supporting activities, generate and distribute monthly care plan calendars, and conduct care plan conferences to ensure seamless execution.
  • Quality Assurance and Education: Participate in facility management meetings, review resident incidents and accidents, update care plans, and maintain ongoing education to promote quality care and operational efficiency.
  • Professional Development: Maintain current knowledge of the RAI Manual and MDS assessment process through attending in-service training and maintaining confidentiality and resident rights.

Compensation and Benefits: Enjoy a competitive salary ranging from $65,000 to $85,000 per annum, along with a comprehensive benefits package, including a 401(k), dental, health, paid time off, and vision insurance.
