We are seeking an experienced and compassionate Registered Nurse to join our team as a Hospice Case Manager. As a valued member of our interdisciplinary group, you will be responsible for delivering high-quality patient care and providing emotional support to patients and their families.
Job SummaryThe RN Case Manager will assume full nursing responsibility for the delivery of the Plan of Care, continuously evaluating performance and making necessary changes. Key responsibilities include:
- Conducting initial nursing evaluations within 48 hours of referral
- Identifying patient and family needs, re-assessing every 15 days
- Initiating and coordinating the plan of care
- Documenting patient condition, problems, goals, interventions, and response to care
- Collaborating with patient, family, physician, and IDG members
