About the Role

We are seeking a highly skilled and compassionate Care Manager, LVN to join our team at IEHP. As a Care Manager, LVN, you will play a critical role in ensuring the health and well-being of our members by providing coordinated care and support.

Key Responsibilities
  • Work closely with the Plan, IPA medical groups, providers, hospitals, community agencies, and other entities to ensure coordinated care and serve as a resource person for internal and external entities.
  • Collaborate with the RN Care Manager and other members of the care team to support the member in the care management process.
  • Develop and implement care plans that meet the unique needs of each member.
  • Communicate effectively with members, caregivers, and healthcare providers to ensure seamless care coordination.
  • Participate in quality improvement initiatives to enhance the overall quality of care.
Requirements
  • Three (3) or more years of care management experience in a healthcare delivery setting.
  • Current IEHP Team Members who have worked at least two (2) years in the Care Management department and are successful in their current work performance during that time, may be considered for this position in lieu of the minimum one (1) year clinical experience requirement.
  • High school diploma or GED required.
  • Possession of an active, unrestricted, and unencumbered Vocational Nurse (LVN) license issued by the California Board of Vocational Nursing and Psychiatric Technicians required.
  • Valid California Driver's License.
