About the Role

We are seeking a skilled and compassionate Integrated Transitional Care Nurse, LVN to join our dynamic team at IEHP. As a key member of our Integrated Transitional Care team, you will play a pivotal role in ensuring seamless, quality healthcare for our low and rising-risk members.

Key Responsibilities
  • Conduct risk assessments to identify members who require additional support and care coordination.
  • Collaborate with hospitals and Independent Physician Associations to ensure effective communication and coordination of care.
  • Coordinate transitions of care to prevent readmissions and improve health outcomes.
  • Conduct reviews and assessments to identify areas for improvement and develop strategies to address them.

Requirements

To be successful in this role, you will need:

  • Two (2) or more years of Utilization management/case management in a health care delivery setting in Acute or Skilled nursing, with an emphasis on Concurrent Review and Utilization management
  • High School Diploma or GED required
  • Possession of an active, unrestricted, and unencumbered Vocational Nurse (LVN) license issued by the California Board of Vocational Nursing and Psychiatric Technicians required
