About the Role:

We are seeking a skilled Registered Nurse Case Manager to join our team at LHC Group. As a Hospice RN Case Manager, you will play a vital role in delivering high-quality end-of-life care to our patients and their families.

Key Responsibilities:

  • Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
  • Identify patients' and families' physical, psychosocial, and environmental needs, and reassess as needed, no less than every 15 days.
  • Develop and implement individualized plans of care, collaborating with patients, families, attending physicians, and other members of the Interdisciplinary Group (IDG).
  • Document patient progress, problems, goals, interventions, and responses to care in accurate and relevant clinical notes.

Requirements:

  • Current RN licensure in the state of practice and one year of clinical experience.
  • Current CPR Certification.
  • Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
