Registered Nurse Case Manager

As a Registered Nurse Case Manager, you will play a vital role in assessing, planning, implementing, coordinating, monitoring, and evaluating options and services to optimize the health status of patients or members. You will manage utilization and practice metrics to refine the delivery of care models, maximizing clinical, quality, and fiscal outcomes.
Key Responsibilities:
- Develop patient-centric treatment plans that integrate evidence-based clinical guidelines, preventive guidelines, protocols, and other metrics.
- Design systems of care that monitor progress and promote early intervention in acute care situations.
- Collaborate with patients, families, primary care providers, and other healthcare team members to ensure coordination of services.
- Continuously evaluate laboratory results, diagnostic tests, utilization patterns, and other metrics to monitor quality and efficiency results.
- Apply benefits and utilization management to serve as a resource to patients and healthcare teams.
- Maintain required documentation for all case management activities.
- Collect and utilize data to adjust treatment plans when indicated.
Requirements:
Education:
Graduate from a Specialty Training Program in Nursing (Required), Bachelor's Degree in Business Administration/Management (Preferred)
Experience:
Minimum of 3 years of Nursing experience (Required)
Certification(s) and License(s):
Licensed Registered Nurse (Pennsylvania) - RN_State of Pennsylvania
Skills:
Computer Literacy, Critical Thinking, Organizing
