As a key member of our healthcare team, you will conduct comprehensive psychosocial assessments of individuals seeking services at our health center.

Key Responsibilities
  • Conduct thorough psychosocial assessments to evaluate patients' needs, including mental health concerns, social determinants of health, and relevant support systems.
  • Collaborate with our interdisciplinary team to gather pertinent information and develop individualized care plans.
  • Provide brief interventions and crisis management as needed during the intake process.
  • Serve as the primary point of contact for patients during the intake phase, offering empathy, support, and guidance.
  • Utilize electronic health records (EHR) systems to accurately document assessment findings and treatment recommendations.
  • Work closely with other LCSWs and mental health professionals to facilitate seamless transitions and referrals for ongoing care.
  • Stay informed about community resources and referral networks to effectively connect patients with additional support services.
Requirements
  • Master's degree in Social Work from an accredited institution.
  • Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of New Jersey.
  • Minimum of 2 years of clinical experience in a healthcare or mental health setting.
  • Strong understanding of diagnostic criteria and evidence-based treatment modalities for mental health disorders.
  • Excellent communication and interpersonal skills, with the ability to establish rapport and build trust with diverse populations.
  • Proficiency in conducting psychosocial assessments and formulating treatment plans.
  • Familiarity with electronic health records (EHR) systems and other technology platforms.
  • Commitment to cultural competence, sensitivity, and trauma-informed care principles.
  • Ability to work collaboratively within a multidisciplinary team environment.
