  • Engage in ongoing communication with families.
  • Coordinate with support networks, including Care Managers and other stakeholders involved in the lives of service recipients.
  • Exhibit flexibility and availability to provide support as needed, which may include occasional weekend or after-hours assistance.
  • Deliver training and guidance to Direct Support Professionals working with your caseload.

Documentation and Reporting:

  • Maintain a daily log to document all services rendered.
  • Submit comprehensive case notes for all billable services on a daily basis into the electronic record-keeping system.
  • Prepare a monthly summary detailing services provided and progress for each individual on your caseload.
  • Participate in Life Plan meetings twice a year for each individual supported, providing feedback and submitting Staff Action Plans.

Work Environment: This role requires a combination of in-office presence and travel to support individuals in their employment settings. A hybrid remote work option is available on non-office days.

  • Strong written and verbal communication skills.
  • Experience in administrative tasks and record-keeping.
  • Proven ability to communicate effectively with team members and external partners.
  • Capacity for active listening and collaboration.
  • Proficiency in computer applications (Google Workspace, Spreadsheets, web-based case notes).
