Key Responsibilities:
- Coordinate home care services and resources for patients transitioning from hospital to home
- Work closely with the healthcare team to ensure a smooth transition of care
- Provide patients and families with support and resources to help them thrive at home
- Collaborate with community resources and agencies to ensure seamless care transitions
Requirements:
- Graduate of accredited nursing or allied health care professional program required
- Licensure in clinical discipline (RN, PT, OT) in the State of Maryland
- Active CPR (Registered Nurse, Physical Therapist, Occupational Therapist)
