Key Responsibilities:

  • Coordinate home care services and resources for patients transitioning from hospital to home
  • Work closely with the healthcare team to ensure a smooth transition of care
  • Provide patients and families with support and resources to help them thrive at home
  • Collaborate with community resources and agencies to ensure seamless care transitions

Requirements:

  • Graduate of accredited nursing or allied health care professional program required
  • Licensure in clinical discipline (RN, PT, OT) in the State of Maryland
  • Active CPR (Registered Nurse, Physical Therapist, Occupational Therapist)
