Key Responsibilities
  • Assess the health status of patients and develop individualized care plans to meet their needs
  • Work collaboratively with multidisciplinary teams to coordinate care and services
  • Provide education and counseling to patients and their families on health maintenance, disease prevention, and condition management
  • Document patient information accurately and thoroughly in electronic medical records
  • Participate in quality improvement initiatives to enhance patient outcomes and care delivery
Requirements
  • Current unrestricted licensure as a Registered Nurse in the state of practice
  • 2+ years of relevant experience in assessing the medical needs of patients with complex behavioral, social, and/or functional needs
  • Ability to work independently and as part of a team in a variety of settings, including non-clinical settings
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations
  • Ability to travel up to 75% of the time for field-based work
