Seeking an experienced Advanced Home Health Care Professional to join our team as a State Traveler across Kentucky. As a key member of our in-home assessment group, you will be responsible for delivering exceptional personalized care to patients in their homes.

Benefits
  • A competitive salary range of $120,000 - $150,000 per year, depending on experience

About the Role

This is a full-time, Monday-to-Friday position with a 4-weeks-on/1-week-off schedule. As a State Traveler, you will work autonomously to see Medicare-aged patients in their homes, performing annual comprehensive health assessments lasting 45-60 minutes. The location varies across the state based on business needs.

Requirements
  • Board Certification in Family Nurse Practitioner (FNP), Acute Care Nurse Practitioner (ACNP), or Adult-Geriatric Nurse Practitioner (AGNP)
  • At least 1+ years of experience as a Nurse Practitioner required
