The selected LPN will be responsible for providing high-quality patient care in a Long-Term Care/Assisted Living setting. The ideal candidate will demonstrate strong clinical judgment and the ability to make informed decisions under supervision.

  • Implement the nursing process to assess, plan, execute, and evaluate patient care
  • Conduct focused interviews to determine specific patient needs
  • Monitor signs and symptoms that indicate changes in patient conditions
  • Gather, analyze, and interpret data to document nursing diagnoses
  • Ensure a safe and therapeutic environment for patients
  • Provide education to patients and/or their families
  • Adjust care plans based on ongoing evaluations
  • Oversee the safety and effectiveness of the care environment and equipment

Qualifications:

  • Valid Licensed Practical Nurse (LPN) License
  • Proficient in English, both spoken and written
  • Current Basic Life Support (BLS) certification
  • Successful completion of required screening assessments
