Job Title: Licensed Practical Nurse/Licensed Vocational Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse/Licensed Vocational Nurse to join our team at STGi. As a key member of our healthcare team, you will provide high-quality patient care and support to our patients in a fast-paced outpatient clinic setting.

Job Summary:

The Licensed Practical Nurse/Licensed Vocational Nurse will work under the direction of the CBOC Clinic Manager, PACT Provider, and STGi Program Manager to provide a wide range of clinical and administrative nursing care. This includes assessing patient conditions, administering medications, performing various nursing procedures, and documenting patient information in medical records.

Responsibilities:
  • Assess patient conditions and develop individualized care plans
  • Administer medications and perform various nursing procedures
  • Document patient information in medical records and VISTA
  • Participate in staff meetings and PACT huddles
  • Provide health education and resources to patients and their families
  • Assist in maintaining the VISTA clinic reminder tracking system
  • Document all pertinent patient information in their medical records and VISTA
  • Assist in scheduling patients for initial and follow-up appointments
  • Provide exceptional customer service and maintain confidentiality of patient information
Requirements:
  • Minimum of two (2) years of current clinical experience working in a similar environment
  • Knowledge of basic methods and procedures and complex methods for coordinating and scheduling clinical care
  • Thorough knowledge of the variations in anatomy
  • Knowledge and ability to apply the practical nursing principles, procedures, and techniques
  • Ability to assess patient condition, recognize adverse signs and symptoms, and react swiftly in emergency situations
  • Ability to follow orders and work under close supervision, while demonstrating initiative and independence
Working Conditions/Physical Requirements:

The Licensed Practical Nurse/Licensed Vocational Nurse will work in a typical interior/office work environment and will be required to respond to any medical emergency within the clinic. The position requires moderate physical activity, including handling average-weight objects up to 25 pounds and standing and/or walking for more than four (4) hours per day.
