Key Responsibilities- Supervise and direct Certified Nurses Aides in providing resident care
- Collect and monitor patient care data, including vital signs and medical history
- Develop and implement individualized care plans in collaboration with the healthcare team
- Administer medications and treatments as prescribed by physicians
- Document patient care and progress in accordance with facility policies and procedures
- Current state license to practice as a LPN
- Minimum 2 years of experience in acute care or rehabilitation setting
- Current CPR certification and ACLS certification preferred
