Job Title: Nurse Practitioner

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at PermiaCare. As a key member of our healthcare team, you will provide high-quality patient care and services to individuals with mental health needs.

Job Summary:

The Nurse Practitioner will be responsible for providing psychiatric services to chronically mentally ill patients in our clinics. This includes performing evaluations, medication maintenance and monitoring, hospital consultations, crisis intervention, and documentation. You will also facilitate Medicaid eligibility determinations, make referrals for and coordinate the delivery of Medicaid services, and provide clinical supervision and guidance to casemanagement and other direct care staff.

Responsibilities:
  • Perform psychiatric evaluations and provide medication maintenance and monitoring
  • Provide hospital consultations and evaluations of OPC patients
  • Document patient visits in a timely manner
  • Provide crisis intervention and attend staff meetings as required
  • Consult with staff about patients and participate in patient treatment planning
  • Complete Patient Assistance Applications in a timely manner and meet unit performance measures or targets
  • Maintain assigned caseload of individuals with mental illness and coordinate services to designated caseload
  • Enter accurate and appropriate documentation of services within timeframe required
  • Maintain confidentiality of sensitive records and treatment information, client files and protected health information in compliance with HIPAA, laws, rules and regulations, and established procedures
Requirements:
  • Master's or post-master's degree from an accredited nurse practitioner or physician assistant program
  • Certification in the state of Texas as a Nurse Practitioner or Physician Assistant
  • License to practice psychiatric medicine in Texas
  • At least 1 year experience prescribing psychiatric medication in an outpatient and/or inpatient setting
  • Valid Texas Driver's license, auto liability insurance, and acceptable driving record
  • Ability to pass criminal history and background checks, as well as pre-employment drug screen
Working Conditions:

This position requires travel to agency program sites, community and residential sites, and/or locations outside the PermiaCare catchment area. You may be required to transport agency individuals and/or individuals served.
