Key Responsibilities:
- Conduct comprehensive in-home assessments, including medical history reviews, medication reconciliation, and physical exams.
- Perform evidence-based practice screenings, including point-of-care testing as needed.
- Identify diagnoses and develop care management plans to address patient needs.
- Communicate findings with patients' primary care physicians to ensure coordinated care.
- Educate patients on disease management, medication adherence, and healthy lifestyle choices.
Requirements:
- Active NP license and unencumbered licensure in the state of assignment.
- ANCC or AANP national certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care.
- Ability to complete physical demands of the job, including lifting, navigating stairs, and sitting/standing/kneeling.
- Reliable transportation for home visits.
Preferred Qualifications:
- 1+ years of clinical experience in a related field.
- Additional licensure in other geographic areas.
- Experience in gerontology, cardiology, internal medicine, or endocrinology.
- Home health care or home visit experience.
