We are seeking a highly skilled MDS Coordinator to join our team at Regency Care of Silver Spring.


Job Summary:

The successful candidate will be responsible for coordinating and maintaining accurate and compliant Minimum Data Set (MDS) documentation for all residents in our healthcare facility.


Key Responsibilities:

  • Conduct thorough resident assessments to capture their medical and social needs
  • Collaborate with the interdisciplinary team to ensure timely and accurate documentation of patient information
  • Review and update MDS documentation as needed to reflect changes in patient status and progress
  • Monitor and ensure accuracy of MDS and care plans in accordance with state and federal regulations
  • Provide training and education to staff on proper MDS coding and documentation guidelines
  • Communicate effectively with families and caregivers to gather information and update them on resident progress
  • Participate in facility meetings and committees related to MDS documentation and compliance
  • Stay up-to-date with changes in MDS regulations and implement necessary changes in documentation procedures

Requirements:

  • Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of Maryland
  • Minimum of 2 years of experience working as an MDS Coordinator in a long-term care facility
  • Deep understanding of MDS documentation requirements and state/federal regulations
  • Strong attention to detail and accuracy in record-keeping
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Proficient in computer skills and electronic medical record systems
  • Familiarity with PointClickCare software is a plus
