Optum House Calls Nurse Practitioner Opportunity

We are seeking a skilled Nurse Practitioner to join our House Calls team, providing annual clinical assessments to patients in the comfort of their homes. This role offers a unique opportunity to make a meaningful impact on patients' lives while enjoying a flexible schedule and work-life balance.

Key Responsibilities:
  • Conduct in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing as appropriate.
  • Identify diagnoses to inform care management and active medical management.
  • Communicate findings to inform members' primary care physicians of potential gaps in care.
  • Intervene in urgent and emergent situations as needed.
  • Educate members on disease processes, medication, and compliance.
Requirements:
  • Active NP license or ability to obtain by start date, with unencumbered licensure.
  • APCs must have obtained approval to practice autonomously or without formal supervision, if applicable.
  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or ability to obtain by start date.
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions.
  • Reliable transportation for home visit assessments.
Preferred Qualifications:
  • 1+ years of clinical experience in the highest level of education, clinical setting.
  • May be requested to obtain additional licensure in other geographic areas.
  • Experience in gerontology, cardiology, internal medicine, or endocrinology.
  • Home health care or home visit experience.
  • Ability to travel throughout the state, beyond the assigned region, up to 5% of the time.

Hawaii Residents Only: The salary range for this role is $88,000 to $173,200 annually. Pay is based on several factors, including local labor markets, education, work experience, certifications, and more. UnitedHealth Group complies with all minimum wage laws as applicable. In addition to your salary, UnitedHealth Group offers a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution, subject to eligibility requirements.
