Job Summary:

As a Geriatric Nurse Practitioner at Kaiser Permanente, you will play a vital role in coordinating case management for members residing in community long-term care facilities. Your expertise will ensure high-quality care, including assessment, evaluation, development, facilitation, and implementation of individualized plans of care. You will provide consultation and education to other healthcare providers, collaborate with special work teams to develop programs supporting continuity of care, and provide consultative and clinic nurse specialist/nurse practitioner services for homecare patients.

Key Responsibilities:
  • Provide consultation and direct care to geriatric inpatient, outpatient, and home care patients.
  • Provide primary care and case management to a panel of members residing in long-term care facilities, including intake assessment, evaluation, development of individualized plans of care, annual physical exams, timely certifications and recertifications, routine follow-up visits, and urgent visits in collaboration with physicians to assess, diagnose, and implement therapeutic plans of care.
  • Encourage and document advanced directives clarifications.
  • Document all care provided to patients, including observations, treatments, medications, and instructions according to Regional and/or facility standards.
  • Work collaboratively with hospitals, clinics, and community sites in program development and development of policies, procedures, quality assessment, and improvement indicators for these programs.
  • Identify needs and coordinate inservice programs, workshops, and seminars.
  • Participate in educational program planning, development, presentation, and evaluation.
  • Act as liaison with and handle facility/family concerns related to long-term care.
  • Identify and report potential problems to appropriate Kaiser Permanente representatives.
  • Participate in patient care conferences with other healthcare team members to coordinate, discuss, and evaluate services provided to patients.
  • Participate in department functions such as staff meetings, staff inservices, and committees as assigned.
  • Represent Kaiser at appropriate community events.
Requirements:
  • Minimum three (3) years direct patient care nursing experience, with one (1) year long-term care or geriatric specialty experience.
  • Masters degree in nursing with specialty in gerontological, adult, family, or acute care.
  • Drivers License (in location where applicable).
  • Nurse Practitioner-Family Certification OR Nurse Practitioner-Acute Care Certification OR Clinical Nurse Specialist Certification-Gerontological OR Nurse Practitioner-Adult-Gerontology Primary Care Certification OR Nurse Practitioner-Adult-Gerontology Acute Care Certification OR Clinical Nurse Specialist Certification-Adult Health.
  • Advanced Practice Registered Nurse License (Hawaii).
  • Registered Nurse License (Hawaii) required at hire.
  • Basic Life Support from American Heart Association.
  • National Provider Identifier required at hire.
Preferred Qualifications:
  • Valid Hawaii APRN Recognition with Prescriptive Authority (must meet education requirement(s) for Hawaii State APRN prescriptive authority).
