RN Case Manager

We are seeking a highly skilled RN Case Manager to join our team at HCA Florida Lawnwood Hospital. As a RN Case Manager, you will play a critical role in ensuring high-quality, patient-centered care by managing rehabilitative care to achieve optimum outcomes.
Key Responsibilities- Provide program orientation to patients, families, and caregivers, including the case manager's role, rehabilitation philosophy, and continued stay and discharge criteria.
- Act as the coordinator of patient, family, and caregiver education, promoting participation in team discussions related to plans, goals, and status.
- Ensure the implementation of the patient's treatment plan, supporting the patient's strengths, abilities, needs, and preferences.
- Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of the patient, family, and caregiver.
- Promote consistent, positive patient interactions that advance the agenda of unparalleled patient service.
- RN with current state licensure; BSN preferred.
- 5 years or more of nursing experience, with 2 years critical care or 3 years nursing experience with 2 years case management experience preferred.
- Certification in case management, nursing, or utilization review preferred.
- InterQual experience, preferred.
- Basic Cardiac Life Support must be obtained within 30 days of employment start date.
HCA Florida Lawnwood Hospital is a 380-bed acute-care hospital offering a full range of services, including labor/delivery, pediatrics, orthopedics, oncology, nuclear medicine, adult and pediatric emergency services, and diagnostic services. We are a designated Certified Primary Stroke Center, with a Physical Rehabilitation Center that is CARF accredited. We are a Level II Trauma Center, verified by the American College of Surgeons, and offer a freestanding ER in Vero Beach. We provide emergency treatment for adults and children, as well as the area's only Level III Neonatal and Pediatric Intensive Care Units.
