Job Summary:

Liberty Health is seeking an experienced Speech Language Pathologist to join our team of healthcare professionals. As a Home Care Speech Language Pathologist, you will provide patient care services and direct treatment to patients in their homes, working closely with our interdisciplinary team to develop and implement individualized plans of care.

Key Responsibilities:

  • Evaluate and treat patients with speech, language, swallow, or cognitive disorders, developing plans of care that address their unique needs and goals.
  • Provide direct patient care services, including assessment, treatment, and follow-up, to ensure optimal patient outcomes.
  • Document patient care data accurately and thoroughly, reflecting evidence of improvement, progress, or decline in patient condition.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination and collaboration.
  • Participate in patient care conferences, meetings, and in-services as scheduled, providing weekly statistical data and contributing to the development of best practices.
  • Provide education and training to healthcare professionals, students, and peers as needed, promoting excellence in patient care and professional development.

Requirements:

  • Master's degree in Speech Language Pathology and Certificate of Clinical Competence from the American Speech-Language-Hearing Association (ASHA).
  • Current licensure in the state where employed by Liberty Health.
  • Strong knowledge of speech language pathology theory and concepts, with the ability to evaluate and treat patients with complex needs.
  • Excellent communication and organizational skills, with the ability to work independently and as part of a team.
  • Minimum of 12 hours of continuing education annually, with a commitment to staying current with best practices and research.
  • Ability to travel and work in a variety of settings, with flexibility to adapt to changing patient needs and priorities.
  • At least 1 year of experience as a Speech Language Pathologist in a healthcare setting.
