Action Wellness seeks a dedicated Professional HIV Disease Case Manager to provide high-quality services to individuals and families affected by HIV/AIDS. This challenging role offers a unique opportunity to make a meaningful difference in the lives of those in need.

About the Position

We are looking for an experienced case manager with expertise in HIV disease management, client advocacy, and community outreach. As a key member of our Direct Services team, you will be responsible for assessing clients' needs, developing individualized care plans, and coordinating medical, mental health, and support services.

**Key Responsibilities:**

  • Act as a medical case manager for individuals and families infected or affected by HIV/AIDS.
  • Conduct thorough assessments of clients' needs and develop comprehensive care plans.
  • Link clients to essential medical, mental health, and support services.
  • Coordinate and monitor services, including advocating for entitlements and benefits.
  • Provide on-site case management in clinics or hospitals as needed.

**Requirements:

  • Minimum Bachelor's degree in Social Work, Human Services, Sociology, Psychology, or Nursing; Master's preferred.
  • Expertise in case management, client advocacy, and community outreach.
  • Ability to work with diverse populations and critical/emergency situations.
  • Understanding of medical and psychosocial issues related to HIV disease and related conditions.

**Benefits Package:**
Action Wellness offers a competitive salary range of $43K-$48K annually, depending on experience, plus excellent benefits, including medical, dental, vision, 401(k), and paid time off.
