Job Overview

Responsibilities
  • Treatment Adherence: Collaborate with our primary care team to encourage patients to adhere to treatment plans, including referrals for identified barriers related to social determinants of health.
  • Patient Outreach: Use various methods to locate and re-engage patients who have missed appointments or are lost to care, developing strong relationships with community health agencies to facilitate ongoing support.
  • Resource Connection: Identify and connect patients with necessary resources, including HIV Peer Support Groups, behavioral health services, and lifestyle medicine classes.
Requirements
  • Education: High School Diploma
  • Language Skills: Spanish language proficiency is preferred but not required
  • Interpersonal Skills: Solid communication and interpersonal skills, including ability to handle challenging situations and build trust with patients
