Position Overview:
The RN Care Manager I - IPCM plays a vital role within the interdisciplinary team, responsible for evaluating, planning, advocating, and coordinating patient care from the point of admission through to discharge. This position is essential in ensuring a seamless transition after hospitalization. Key responsibilities include:
- Assessing patients' health requirements
- Creating personalized care plans
- Coordinating services to enhance health outcomes
Effective communication with patients, families, and healthcare professionals is crucial in this role. The RN Care Manager I is also instrumental in advocating for patients' needs, promoting health education, and facilitating smooth transitions across various healthcare environments. This position demands clinical expertise, critical thinking abilities, and a compassionate demeanor to support patient well-being and improve healthcare results. Responsibilities will be executed in alignment with Mercy's mission, values, and service standards.
Qualifications:
Education:
Graduate from an accredited School of Nursing.
Experience:
A minimum of 1 year in an acute care hospital setting.
Licensure:
Current RN License in the state of employment.
Certifications:
BLS (Basic Life Support) certification required at hire or within 90 days of employment.
Preferred Certification:
Certification in Case Management.
Preferred Experience:
2 years in an acute care hospital setting, with experience in Care Management or Utilization Management.
