Our team of experienced professionals works closely with clients to understand their unique needs and develop personalized care plans that promote independence and community integration.

Job Summary

The Support Coordinator/Service Coordinator/Case Manager plays a vital role in our team, responsible for providing case management activities that meet the complex needs of our clients. This includes developing individualized care management plans, brokering and coordinating services, and monitoring and evaluating clinical outcomes to ensure the highest level of care.

Key Responsibilities
  • Conduct comprehensive assessments to identify client needs and preferences for service delivery.
  • Develop and implement Person-Centered Service Plans that reflect client goals and aspirations.
  • Ensure client choice of providers by providing accurate and timely information.
  • Coordinate with other care professionals to ensure seamless delivery of physical, behavioral, and support services.
  • Research and develop alternative solutions to meet client needs.
  • Maintain confidentiality and handle sensitive information with discretion.
  • Attend required trainings and recertification classes to stay up-to-date on best practices.
  • Collaborate with supervisors and directors to ensure alignment with organizational goals and objectives.
Qualifications
  • Bachelor's degree in a related field.
  • Minimum 3 years of experience in case management for individuals with Intellectual Disabilities, Autism Spectrum Disorders, or Physical Disabilities.
  • Proficiency in Microsoft Office and state-maintained databases.
  • Maintain CPR/First Aid certification and updated state-related clearances.
  • Valid Driver's License with personal vehicle insurance coverage and current vehicle registration.
