About This Opportunity

CareATC is seeking a licensed Mental Health Professional to join our team as a patient advocate. In this role, you will work collaboratively with the primary care/interdisciplinary treatment team to provide patient-centered care.

Key Responsibilities
  • Provide evidence-based individual counseling to patients within a mental health or healthcare facility.
  • Assess and address patient needs, including social determinants of health, to ensure continuity of care.
  • Collaborate with the healthcare team to develop and implement patient-centered care plans.
  • Provide psychoeducation to patients and employers to promote health and wellness.
  • Identify and address barriers to care, including insurance benefits and community resources.
Requirements
  • M.S.W. or equivalent degree from a graduate school of social work fully accredited by the Council on Social Work Education (CSWE).
  • Current license with the Board of Social Work (LCSW, LPC) or equivalent license or related field.
  • At least 12 months of experience in assessing and providing evidence-based individual counseling to patients within a mental health or healthcare facility.
  • At least 12 months of direct hands-on experience with computer operations, basic word processing, data entry, and use of an automated medical record system.
