About the Role:

The Licensed Practical Nurse (LPN) is a vital member of our healthcare team, providing high-quality nursing care to patients and families under the supervision of a Registered Nurse (RN) or provider. As an LPN, you will apply your nursing knowledge and skills to deliver evidence-based care, ensuring the best possible outcomes for our patients.

Key Responsibilities:

  1. Maintain competency and practice according to the Gundersen Health System mission and strategic plan, ANA, and Nurse Practice Act.
  2. Contribute to assessments through data collection, observing and reporting patient responses to care, and significant symptoms or changes in patient status.
  3. Follow the Patient's Plan of Care, including the Clinical Practice Model (CPM) and standard collaborative practice, and perform delegated patient care.
  4. Incorporate patient/family readiness to learn when reinforcing teaching initiated by an RN or provider, achieving optimal, age-specific, and individualized care.
  5. Provide direct patient care, assist with ADLs, encourage self-care, and administer prescribed treatment after verifying accuracy.
  6. Document nursing interventions and patient/family response, ensuring accurate and timely documentation in compliance with organizational, regulatory, and professional standards.
  7. Participate with other health team members in meeting basic patient needs, without delegating tasks.
  8. Perform other job-related duties as assigned.

Education and Learning:

Post-high school education in a specialized field is required, with course requirements qualifying to take LPN or LVN boards.

Work Experience:

No experience is required, but desired experience as a LPN or in a healthcare environment is preferred.

License and Certifications:

A Licensed Practical Nurse (LPN) license is required, with some positions requiring additional state LPN licenses. Current Infant-Adult Basic Life Support for Health Care Providers certification is also required.

Age-Specific Population Served:

Infant (less than 1 year), Pediatric (1-12 years), Adolescent (13-19 years), Adult (20-64 years), and Geriatric (65 years & older) populations are served.

OSHA Category:

Category I - All Employees in this job title have a reasonably anticipated risk of occupational exposure to blood and/or other potentially infectious materials.

Environmental Conditions:

Exposed to infectious diseases.
