Case Manager

We are seeking a skilled Case Manager - Social Work Specialist to join our team at Kaiser Permanente's Gaithersburg Hub. As a key member of our healthcare team, you will work closely with patients, families, and caregivers to provide comprehensive case management services.
Key Responsibilities:- Collaborate with healthcare professionals to develop and implement individualized care plans that promote optimal health outcomes and quality of life.
- Conduct thorough psychosocial assessments to identify patient needs and develop strategies to address them.
- Facilitate access to community resources and services to support patients and their families.
- Provide education and support to patients, families, and caregivers on health-related topics and end-of-life issues.
- Monitor and analyze patient outcomes to identify opportunities for improvement in case management practices.
- Master's degree in Social Work (MSW) required.
- Minimum 3 years of clinical experience and 2 years of case management experience required.
- Licensed Clinical Social Worker (LCSW) certification required within 6 months of hire.
- Experience with computer software programs in a Windows environment preferred.
- Knowledge of community systems and resources in the defined service area preferred.
