**About the Role**
We're seeking a dedicated Hospice Aide/CNA to join our team. As a Hospice Aide/CNA, you'll play a vital role in providing world-class patient care. You'll work closely with our patients and their families to create incredible memories and make a meaningful difference in their lives.
**What You'll Do**
* Provide compassionate care to patients in their homes or in our facilities
* Work collaboratively with our interdisciplinary team to develop and implement patient care plans
* Conduct vital signs, administer medications, and perform other clinical tasks as needed
* Maintain accurate and detailed records of patient interactions and care provided
