As a top Recruitment Process Outsourcing provider, Strong Force Staffing is currently seeking an RN Case Manager for a respected client agency in Glendale.

Key Details:

  • Salary: $82,500 - $97,800 Per Year, Depending on Experience
  • Benefits: On-call stipends, Mileage reimbursement

Responsibilities:

  • Assess patient conditions and develop tailored care plans.
  • Maintain detailed medical records and communicate patient status to relevant parties.
  • Serve as the primary liaison among patients, their families, and healthcare providers.
  • Educate patients and families on health management and preventive measures.
  • Coordinate with healthcare professionals to ensure continuous care and efficient case management.

Requirements:

  • License: Active RN license in Arizona with no restrictions.
  • Experience: At least 1 year in a similar in-home healthcare role.
  • Skills: Proficient in case management software, exceptional organizational and communication skills.
  • Traits: Empathetic, detail-oriented, and able to operate autonomously in field settings.
