About the Role

Canyon Home Care & Hospice is seeking a skilled Speech Language Pathologist to join our team in West Valley. As a Speech Language Pathologist, you will administer speech therapy to patients in their place of residence, working in accordance with physician orders and plan of care under the direction and supervision of the Clinical Manager/Therapy Supervisor.

Responsibilities

• Assist the physician or intermediate care provider in evaluating level of function, helping develop the plan of care and revise as necessary.

• Prepare clinical and progress notes.

• Advise and consult with the family and other agency personnel.

• Participate in in-service programs and present in-service programs as assigned.

• Understand and adhere to established policies and procedures.

• Participate with all other health care personnel in patient care planning.

• Complete initial history and evaluation visit, notify physician of patient needs and submit plan of care and orders for physician approval.

• Perform all skilled Speech Language Pathology services as ordered by physician.

• Consult with physicians regarding change in treatment.

• Write reports to physicians regarding patients progress.

• Attend all staff meetings and patient care conferences as scheduled.

Requirements

• A person who has a masters or doctoral degree in speech-language pathology, and who meets either of the following requirements.

• Is licensed as a speech-language pathologist by the state in which the individual furnishes such services.

OR

• In the case of an individual who furnishes services in a state which does not license speech-language pathologists:

• Has successfully completed clock hours of supervised clinical practicum (or is in the process of accumulating supervised clinical experience);

• Performed not less than 9 months of supervised full-time speech-language pathology services after obtaining a masters or doctoral degree in speech-language pathology or a related field; and

• Successfully completed a national examination in speech-language pathology approved by the Secretary.

• Two years experience, preferred.

• Acceptance of philosophy and goals of Agency.

• Ability to exercise initiative and independent judgment.
